Attribution rules
A Devoted MA member is attributed to Caliber when all three conditions are met within 12 rolling months:
- ≥2 ICD-10 dx for SMI (F20–F29, F30–F31, F32–F33 recurrent, or F25)
- Active Devoted MA enrollment in NC/SC/GA
- Not opted out of supplemental care management (member consent on file)
Attribution is verified monthly. Disenrollments are removed within 30 days of effective date for PMPM purposes.
